Community Hospice

For over 36 years, Community Hospice has walked along side patients and families as they have navigated life’s end-of-life journey. Founded in a church basement by two volunteer registered nurses, Community Hospice has grown to meet the needs of the community becoming the oldest and largest non-profit hospice provider in the San Joaquin Valley.
Community Hospice’s mission is to provide compassionate and quality care, education and support to terminally ill patients and their families, regardless of ability to pay. Although many insurance companies do have some reimbursement for hospice care, Community Hospice provides more than $500,000 a year in charity care as well as offering additional support programs. Services extend beyond hospice care, offering several educational opportunities throughout the year, as well as free grief support to anyone in the community that has experienced a loss. Through individual/family counseling, grief support groups, school grief programs, Camp Erin® Modesto and other programs, Community Hospice is always available in your time of need.
Through the dedication and fundraising efforts of the Community Hospice Foundation, Community Hospice Foundation Friends of Hospice, Community Hospice Hope Chest Thrift Stores and many generous community donors, all services and programs are offered regardless of ability to pay…that’s our promise!
